中文摘要 | To investigate the effects of a beta-adrenergic agonist on pulmonary hemodynamics in patients with decompensated emphysema, we prospectively studied 46 patients using echocardiography. The patients were randomized into two groups, those receiving inhaled terbutaline (Group T, n=26) and the control group receiving saline inhalations (Group P, n=20). After the initial nhalation, patients in Group T had a significant reduction in their respiratory rate (23±4 to 16±5 /min, p<0.001) and heart rate (122±21 to 104±18 /min, p<0.01). There was a concomitant increase in arterial oxygen partial pressure (58±8 to 70±11 mmHg, p<0.001), reduction in carbon dioxide partial pressure (56±9 vs. 48±8 mmHg, p<0.01), and increase in oxygen saturation in Group T. There were no such improvements in Goup P. In addition, stroke volume increase (51±10 to 60±8 mL, p<0.001) and pulmonary artery pressure decreased (37±10 to 28±8 mmHg, p<0.001) significantly after terbutaline inhalation. In conclusion, terbutaline, as a beta-adrenergic agonist, appears to improve pulmonary hemodynamics via its bronchodilatory and vasodilatory effects. |
英文摘要 | 為研究乙型交感神經興奮劑吸入治療對失償性肺氣腫病患的肺部血行動力學的影響,我們對46位病患進行心臟超音波的前瞻性評估。這些病患被隨機分為接受terbutaline吸入治療(T組,共26人)以及只接受saline吸入治療(P組,共20人)作為初始治療的兩組。在吸入治療後,T組病患的呼吸頻率(23±4 至 16±5 /min, p<0.001)以及脈搏數(122±21 至 104±18 /min, p<0.01)都有顯著的下降。同時T組病患的動脈氧分壓上升(58±8 至 70±11 mmHg, p<0.001),二氧化碳分壓下降(56±9 至 48±8 mmHg, p<0.01),以及動脈氧飽和度的增加。而以上現象在P組皆未發生。此外,T組病患在吸入治療後,心搏出量也有顯著增加(51±10 至 60±8 mL, p<0.001),而肺動脈壓則有明顯地下降(37±10 至 28±8 mmHg, p<0.001)。因此我們相信terbutaline這種乙型交感神經興奮劑,可以藉由其支氣管擴張以及血管擴張作用,改善失償性肺氣腫病患的肺部血行動力學。 |
