中文摘要 | 本研究的目的是使用螺旋槳式成像方式改善擴散加權影像於大腦顳葉之影像品質及增進膽脂瘤之診斷能力。針對23 位罹患中耳膽脂瘤的病患使用3T磁振造影進行檢查,比較傳統單次激發面迴訊脈衝序列和使用螺旋槳式成像技術擴散加權影像在大腦顳葉的影像品質以及其對於膽脂瘤之診斷。結果顯示使用螺旋槳式成像的技術比傳統使用單次激發面迴訊脈衝序列擴散加權影像有較少的磁化率假影以及較輕微的幾何變形(P<0.01),對於膽脂瘤的診斷有較高的敏感性及準確度(P<0.01)。因此使用螺旋槳式成像之擴散加權影像可以幫助對於膽脂瘤病患的診斷價值。 |
英文摘要 | The purpose of the study was to improve the image quality of diffusion weighted imaging (DWT) in temporal lobe and investigate the diagnostic utility for patients with cholesteatoma. Twenty-three patients underwent a 3 Tesla MRI. Two techniques of DWI, which include conventional single-shot Echo Planar Imaging (SSEPI) and the Periodically Rotated Overlapping Parallel Lines with Enhanced Reconstruction (PROPELLER) DWT, were compared for the image quality and the diagnostic ability. The results showed that the susceptibility artifact and the geometric distortion were significantly reduced in the area of temporal lobe using PROPELLER DWT compared with those using SSEPI DWI (P<0.01). The sensitivity and the diagnostic accuracy in detecting cholesteatoma were improved using PROPELLER DWI compared with those by using SSEPI (P<0.01). It is concluded that the PROPELLER DWT could aid the diagnostic utility in patients with cholesteatoma. |
