中文摘要 | 院內感染的發生因醫院的性質及區域性的不同而有所差異。我們針對一區域醫院由1995至1997年的院內感染資料加以整理分析。該院共約527床,1995至1997年共有82,547人次出院,依院內感染定義收案院內感染人次共2,386,三年平均感染率為2.9%,各部位感染依序為:泌尿道28.9%、呼吸道23.4%、血流19.2%、外科傷口16.6 %,其它部位10.6%、皮膚皮下1.2%。院內感染致病菌以革蘭氏陰性菌最多佔68.8%、革蘭氏陽性菌佔18.7%、黴菌佔12.5%。院內感染病原菌前五名依序為:Pseudomonas aeruginosa 16.2%、yeast-like organism 12.5%、Staphylococcus aureus 9.8%、Acinetobacter spp. 8.6%、Klebsiella pneumoniae 8.4%;其中yeast-like organism高居第二,顯示可能有抗生素使用繁多情形,而Acinetobacter spp.與K. pneumoniae亦有逐年上升趨勢,需特別注意。 |
英文摘要 | Nosocomial infection surveillance data from 1995 till 1997 at our 527-bed hospital were analysed. Trained infection control nurses visited all wards twice a week and scanned charts to search for patients with evidence of nosocomial infections. The case difinitions followed those of United States Centers for Disease Control and Prevention. There were a total of 82,547 admissions during this period with 2.386 hospital-acquired infections, with an annual infection rate of 2.9%. Infection rates of individual sites were: urinary tract 28.9%, respiratory tract 23.4%, blood stream 19.2%, surgical wound 16.6%, skin and adjacent tissues 1.2%, and others 10.6%. Pathogens were Gram-negative bacteria 68.8%, Gram-positives 18.7% and fungi 12.5%, in the following order of frequencies of each organism: Pseudomonas aeruginosa 16.2%, yeast-like organisms 12.5%, Staphylococcus aureus 9.8%, Acinetobacter spp. 8.6%, Klebsiella pneumoniae 8.4%. Acinetobacter spp. and Klebsiella pneumoniae infections appear to be increasing over these three years, 5.5% to 12.4% and 5.4% to 9.7%, respectively. It is noteworthy that the yeast-like organisms were the second in frequency, probably reflecting the incessant use of antimicrobials in the hospital. |
