中文摘要 | 典型心房撲動是一種心房性的心律不整,有鋸齒狀P波的心電圖特徵,而且心房跳動的頻率為每分鐘 240 至 340 次。典型心房撲動存在兩種型態,即逆時鐘迴轉型與順時鐘迴轉型。目前經研究得知,逆時鐘迴轉型心房撲動的出現是因為在右心房內存有一個與解剖構造相關的大迴路。去極化波沿著房中隔上行,再沿著右心房側壁往下傳導,週而復始。由下腔靜脈入□、歐氏脊、冠狀靜脈竇入□與三尖瓣環所圍成的下右心房峽部是整個迴路的慢速傳導區,也是形成心房撲動的關鍵所在。順時鐘迴轉型的心房撲動其去極化波的傳導順序與逆時鐘迴轉型的心房撲動正好相反,而且週期長度相似。輻射頻率導管灼燒右心房峽部,造成雙向傳導阻斷,能安全又有效治癒典型心房撲動。 |
英文摘要 | Typical atrial flutter is classically defined as an atrial arrhythmia at a rate of 240-340 beats/min with a saw-tooth ECG pattern. Typical atrial flutter exists in two forms: counterclockwise and clockwise. It is now known that counterclockwise atrial flutter is due to an anatomically defined macroreentrant circuit contained within the right atrium. The depolarization wavefront travels up the interatrial septum and down the right free wall. An important element in the circuit is a zone of slow conduction in the isthmus bounded by the tricuspid ring, the inferior vena cava, the Eustachian ridge, and the coronary sinusostium. Clockwise atrial flutter has a reverse activation sequence of counterclockwise atrial flutter. Radiofrequency catheter ablation of the right atrial isthmus with creation of bidirectional conduction block is safe and effective in treatment of typical atrial flutter. |
