中文摘要 | 本研究的目的是探討以三度空間-電腦斷層影像重組技術評估早期顱縫骨化症及其所合併的腦內異常之角色。共有14例臨床上疑有早期顱縫骨化症的患者接受CT檢查,CT機器是Picker PQ2000螺旋狀CT。我們使用螺旋狀技術做CT切面,再於Voxel Q工作站上做頭顱的三度立體影像重組,並以此影像評估顱縫及頭顱外形。而原CT影像又可用來評估腦內有無異常。早期顱縫骨化症可以分為三類,本研究14名患者中有8人屬於第一類(單一種顱縫骨化),包括6例metopic suture骨化,其中4例合併三角頭畸形;1例雙側冠狀縫骨化,合併扁頭;1例lambdoidal suture骨化。第二類(二種以上顱縫骨化)有3例,其中1例合併前腦發育不良症。第三類(所有顱縫骨化)有3例,他們都合併極嚴重的腦萎縮,其中1例合併前腦發育不良症。第三類(所有顱縫骨化)有3例,他們都合併極嚴重的腦萎縮,其中1例是Seckel症狀群的患者。結論:三度空間-電腦斷層影像重組可以很清楚地觀察顱縫,而且它最大的好處是可以在螢幕下旋轉各種不同的角度,檢視所有的顱縫,也可順便檢視腦組織有無異常。 |
英文摘要 | The purpose of the study is using three dimensional (3D) computed tomography (CT) reconstruction of cranium for evaluation of craniosynostosis and their associated brain anomalies. Fourteen patients who were suspected craniosynostosis clinically were referred for CT study. The CT was a Picker PQ 2000 spiral CT, continuous scanning were performed using spiral technique, and 3D images were reconstructed by Voxel Q workstation. Source views of axial CT, were used for evaluation of brain condition. Three patients also received magnetic resonance imaging (MRI) study. There are three categories of craniosynostosis. Category I was single suture synostosis, included six patients of metopic suture synostosis, four of them presented as trigonocephaly; one patient of bilateral coronal suture synostosis which presented with brachycephaly; one patient had bilateral lambdoidal synostosis. Category II included three patients of complex sutures (more than two suture synostosis), one of them combined holoprosencephaly. Category III included three patients of early closure of all the cranial sutures, all of them had significant brain atrophy, and one patient had Seckel syndrome. We found that 3D CT provided good surface images of the cranium, and could be observed the cranial sutures by rotating various directions; its source views of CT of the brain could be used for observation the brain condition, so 3D CT is the most optimized diagnostic modality for evaluation of craniosynostosis. |
