中文摘要 | 根據衛生署統計心臟疾病是國人十大死因之一,其中冠狀動脈阻塞佔了大部份。目前對於冠狀動脈阻塞的治療大多施以血管擴張術(angioplasty)或血管繞道手術(surgical bypass grafting),近年發展的經皮窠透血管擴張術(percutaneous transluminal coronary angioplasty,PTCA)及抗阻塞器(steot)置入方法亦對冠狀動脈治療助益頗多。但這些方法施行後不可避免的會對血管管壁造成一些損傷,而這些損傷導致局部發炎或出血因而使血液基質、單核巨噬細胞及淋巴球在此累積,造成對管壁平滑肌細胞的刺激而使之遷移及增殖,如此過度的反應使血管內膜增厚因而造成血管再次阻塞。近年的一些臨床試驗發現血管內近接放射線治療(endovosclar brachytherapy)對血管再狹窄(restenosis)的預防有顯著的功效,可以成為一種對心臟血管疾病有用的治療方法。目前對於反應機制、施行方法與血管對輻射劑量反應尚有許多需研究與改進之處,本文將對此做一探討。 |
英文摘要 | Cardiovascular disease is one of ten major causes of death in Taiwan. Coronary artery stenosis is the major entity of cardiovascular disease. Usually such coronary artery disease is treated with coronary artery angloplasty or coronary artery bypass grafting. However, disruption of intima and media of artery following percutaneous transiuminal coronary angioplasty and stenting technique would lead to inflammatory cell infiltrate of mononuclear macropha-es and lymphocytes. As a result, stimulation of smooth muscle cell to migrate and proliferate would lead to neointima hyperplasia and restenosis. In recent years, several trials demonstrate that endovascular brachytherapy play an effective role for prevention of restenosis. The mechanism of restenosis is a complex, multifactorial process. Further research for improve of delivery system and design of radioactive sources is needed. |
