中文摘要 | 本文描述以Watson關懷理論照護低位直腸癌接受永久性腸造口病患的護理經驗。筆者自2012年06月10日至20日護理期間,藉由觀察、會談、傾聽及身體評估的方法收集資料,確立主要護理問題有皮膚完整性受損、身體心像紊亂和預期性哀傷等。提供個別的護理措施,包括:支持、關懷、協助表達內心感受與哀傷情緒、介紹成功個案增加信心;教導個案及家屬腸造口、傷口照護,使個案重新接受身體外觀的改變,進而參與學習自我照顧;轉介資源,協助後續照護,使個案身、心、社會各層面需求達到滿足。對接受永久性腸造口且住院期短的病患,學習並照護與自己共存的新構造,極不容易,建議臨床護理人員可多加運用此關懷理論,照顧這類的病患。 |
英文摘要 | It is important to help the low rectum cancer patients with permanent colostomy in a short hospital stay to reduce their physical and mental burdens. This article presents the application of Watson's Human Caring Theory to a low rectum cancer patient with permanent colostomy. From June 10, 2012 to June 20, 2012, data were collected through direct care, listening, observation, accompany, and conversation. Major nursing problems such as "impaired skin integrity," "body image disturbance," and "anticipatory grief" were identified after analysis. With clear and definite nursing targets, we applied Watson's caring theory to provide individual nursing assessments, including support, care, assistance to express feelings and grief emotions, and introduction of the experience of successful cases. We also taught the patient and her family the relevant knowledge about colostomy care, so that the patient could accept her body appearance change and participate in learning self-care. In addition, we provided the social referral resources to satisfy the total care in the physical, mental and spiritual dimensions. It is very difficult for the low rectum cancer patients with permanent colostomy to learn to cope with their new body construction smoothly in a short hospital stay. We recommend that the nursing personnel care these patients using the Watson's Human caring theory in the future, establishing a good nurse-patient relationship with trust first, and then giving them the holistic care of body, mind, and society. |
