中文摘要 | 我們以瓊脂稀釋法測定cephalothin, gentamicin, vancomycin, teicoplanin, fusidic acid,及mupirocin對1990年至1992年間臺大醫院臨床分離出之112株金黃色葡 萄球菌之最低抑菌濃度,另以微量肉湯稀釋法測定這些菌株對oxacillin之感受性。結果其 中58株為methicillin感受性菌株MSSA),54株為methicillin抗藥性菌株(MRSA)。所有菌 株對vancomycin及teicoplanin皆具感受性,最低抑菌濃度分別為0.25~2μg/mL及0.06~2 μg/mL。所有MSSA菌株對cephalothin, fusidic acid,及 mupirocin皆具感受性,最低抑 菌濃度分別為0.125~0.5μg/mL,≦0.03~0.25μg/mL,及0.06~1μg/mL。MRSA菌株中94.4% 對mupirocin具感受性,最低抑菌濃度為0.06~4μg/mL,MIC90為0.5μg/mL。Fusidic acid 對MRSA菌株之最低抑菌濃度為0.5~4μg/mL,MIC90為2μg/mL。98.1%的MRSA菌株對 gentamicin具抗藥性。因此,我們得到以下結論:在臺灣地區,除了vancomycin之 外,teicoplanin及mupirocin對MRSA,MSSA也具有很好的抑菌效果。Fusidic acid對MSSA 抑菌效果很好,但對MRSA菌株抑菌效果仍須再評估。臨床醫師對上述抗生素之使甩應加以適 當的節制,以避免或延緩抗藥性之產生。 |
英文摘要 | The minimum inhibitory concentrations (MICs) of cephalothin, gentamicin, vancomycin, teicoplanin, fusidic acid, and mupirocin against 112 strains of Staphylococcus aureus, isolated from various clinical specimens at National Taiwan University Hospital between 1990 and 1992, were determined by agar dilution method. Oxacillin susceptibility was determined by broth microdilution method. Fifty-eight were methicillin-sensitive strains (MSSA), and 54 were methicillin-resistant strains (MRSA). All S. aureus strains were susceptible to vancomycin with the MICs ranging from 0.25 to 2μg/mL. They were all susceptible to teicoplanin too, with the MICs ranging from 0.06 to 2μg/mL. All MSSA strains were susceptible to cephalothin, fusidic acid and mupirocin with the MICs ranging from 0.125 to 0.5μg/mL,≦0.03 to 0.25μg/mL, and 0.06 to lμg/mL, respectively. Among MRSA strains, 94.4% were susceptible to mupirocin with the MICs from 0.06 to 4pg/niL and an MIC90 of 0.5μg/mL. The MICs of fusidic acid against MRSA strains were in the range of 0.5 to 4μg/mL and the MIC90 was 2μg/mL. Almost all MRSA strains (98.1%) were resistant to gentamicin. We conclude that, in addition to vancomycin, teicoplanin and mupirocin have good antimicrobial activities against both MRSA and MSSA in Taiwan. Fusidic acid is active against MSSA, but its activity' against MRSA needs to be evaluated further. These agents should be carefully regulated in clinical use in order to preserve their antistaphylococcal activities as long as possible. |
