| 英文摘要 | Background: The purpose of this study was to evaluate the efficacy of Tc-99m DISIDA cholescintigraphy in monitoring the recovery of gallbladder function after percutaneous transhepatic gallbladder drainage (PTGBD) in patients with acute acalculous cholecystitis. Methid: A total of six patients with acute acalculous cholecystitis were treated by PTGBD. All received a baseline cholescintigraphy with 185 MBq Tc-99m DISIA and a sonographic study to confirm the diagnosis. After PTGBD, each patient was followed monthly with a Tc-99m DISIDA cholescintigraphy, sonography, liver function test and clinical evaluation. Resalts: Three patients were symptom free in three months after PTGBD, and the drainage tubes were then removed. Serial Tc-99m DISIDA studies of these three patients showed visualization of gallbladder radioactivity one to htree months after PTGBD. In three non-improved patients, gallbladder activity was not visualized in any of the follow-up Tc-99m DISIDA studies. Two of these non-improved cases later received cholecystectomy due to recurrent acute cholecystitis; one case showed persistent signs and symptoms of cholecystitis. The recovery of gallbladder function began at one month after treatment. Cholecystectomy should be considered if gallbladder radioactivity is not seen in Tc-99m DISIDA study up to three months after PTGBD. Conclusions: We conclude that visualization of gallbladder radioactivity in Tc-99m DISIA study is a reliable sign of the recovery of gallbladder function in patients who have received PTGBD for treatment of acute acalculous cholecystitis. |
