中文摘要 | 弓蟲感染分佈很廣,在經濟及公共衛生上很重要。由於弓蟲症缺乏特異性臨床症狀,因此必須使用診斷技術偵測感染。本研究乃比較偵測抗原之enzyme-linked immunosorbent assa(ELISA),增幅核酸之polymerase chain reaction (PCR),及測量抗體之kinetics-based ELISA(KELA)及immunoblotting(IB)。所使用之血液樣品採自200隻貓、300隻狗、及100個人。雖然抗原-ELISA及PCR為偵測免疫抑制個體之有效診斷工具,本研究卻發現當使用於一般血液樣品測試時,其靈敏度很差。由於KELA及IB之測試結果頗一致,在KELA及IB皆為陽性者乃定義為「相對的」真正陽性。因此,二者一起使用時,有最好的靈敏度及特異性。KELA及IB在貓、狗、人弓蟲感染診斷之相對靈敏度及相對陰性預測值皆為100%。KELA之相對特異性及相對陽性預測值分別為95.8-99.5%及75.0-90.5%,而IB為93.7-100%及75.0-100%。 |
英文摘要 | Toxoplasma gondii infection is widespread and is of economic and public health impor-tance. Because toxoplasmosis lacks specific clinical signs or syndromes, the application of diagnostic techniques is required to detect the infection. In this study, antigen detection by avidin-biotin enzyme-linked immunosorbent assay (ELISA), nucleic acid amplification by nested polymerase chain reaction (PCR), and IgG or IgM antibody assay by kinetics-based ELISA (KELA) and immunoblotting (IB) were compared using blood samples from 200 cats, 300 dogs, and 100 humans. Although antigen-ELISA and PCR have proved useful as diagnostic tools in immunocompromised hosts with T. gondii infection, this study showed that their sensitivity was poor when used for routine testing of blood samples. Because the diagnostic results of KELA and IB were in high agreement (92.5-99.0%), “relative” true positive was defined as being positive in both KELA and IB. Thus, combined use of both assays would offer the best sensitivity and specificity. The relative sensitivity and relative negative predictive values of KELA and IB in the diagnosis of T. gondii infection in cats, dogs, and humans were 100%. The relative specificity and relative positive predictive val-ues for KELA were 95.8-99.5% and 75.0-90.5%, and for IB 93.7-100% and 75.0-100% respectively. |
