中文摘要 | 本研究的目的在於探討床頭抬高角度改變對腦部手術後病患腦血流之影響,以瞭解床頭抬高後腦血流速度的立即變化,以及抬高10分鐘後腦血流是否可恢復穩定;為前實驗設計研究法,採目的取樣的方式,在一所醫學中心的神經加護病房收集10位因各種因素接受腦部手術的病患,使用經顱杜卜勒超音波分別測量床頭放平、床頭抬高15、30或45時,大腦中動脈的平均血流速度、收縮期尖峰血流速度、舒張末期血流速度的變化,以了解床頭抬高角度改變時腦血流的立即改變。在五個月的收案期間共有10位病患參與,使用單因子變異數分析 (one-way ANOVA) 進行統計上的檢定。結果顯示床頭抬高15、30或45經10秒鐘時,大腦中動脈的平均血流速度、收縮期尖峰血流速度、舒張末期血流速度,均有明顯下降的情形;腦血流在抬高15、30及45第9分鐘時即能恢復穩定。此研究分析各種抬高角度對於腦部手術後病患腦血流的影響,其結果可提供臨床護理人員照顧類似病患時作為參考。 |
英文摘要 | This pre-experimental research measured patients' mean blood velocity (MV), peak systolic velocity (PSV), and end-diastolic velocity (EDV) of the middle cerebral arteries by using transcranial Doppler (TCD) to understand the immediate changes in cerebral blood flow (CBF) when head elevation degree changed, and whether CBF will return to stability after 10 minutes of elevation. Ten patients who underwent cerebral surgery were selected from one medical centre by purposeful sampling within five months. One-way ANOVA was used to analyze the data. The study results showed that MV, PSV, and EDV had obviously changed when head elevation was 15, 30, or 45 from the horizontal at 10 seconds, and then recovered gradually. This study examined four head elevation degrees that affected cerebral blood flow. The results are likely to help nurses in caring for these patients. |
