中文摘要 | 因為年齡增加或疾病因素導致部分患者步態有問題,有時這些步態問題無法完全治癒,因此常見他們會使用例如手杖、拐杖與助行器等助行工具來解決步行問題。 助行工具雖然可以提供如增加穩定度、信心及減少下肢承重的好處,但是它也會帶來一些骨骼肌肉系統與生理負擔。因此臨床人員在建議選用助行工具前若對這些工具的優劣點未詳細了解,可能會導致使用者不利的後果。對時間有限的臨床人員要能夠由患者下肢可承重的程度與平衡能力來決定要選擇哪一種助行工具,並決定工具的高度、手肘彎曲的角度以及以哪一側手持用等實務的問題,需要先了解這些助行工具。在這篇文章裡,我們回顧使用助行工具的流行病學、與各式助行工具的優缺點。其次提出常見的各式助行工具的選用原則,並介紹一些使用上需注意的重點。 |
英文摘要 | Some patients have gait disorders due to aging or disease and most cases cannot be cured. Therefore, treatment often relies on walking aids such as cane, crutch, and walker for locomotion. Walking aids can be used to improve patient's stability, self-confidence, and redistribute weight from a lower extremity, but are not without significant musculoskeletal and metabolic demands. Although they are often prescribed, a detailed understanding of these devices is often lacking and inappropriate selection of a device might lead to detrimental consequences. Because time is often limited during a routine clinical visit, a comprehensive knowledge of walking aids is essential for practitioners to ensure proper height and handedness, and consultations for correct use of the devices. Patient's need to bear weight through the device and balance ability are determinable on clinicians' choose on devices. In this article, we review the prevalence, advantages and possible disadvantages associated with walking aids. We then discuss the core clinical principles for choosing appropriate walking aids and guidelines to using the devices. |
