英文摘要 | Primary adenocarcinoma of the small intestine is a relatively rare neoplasm and, as such, knowledge of its clinical characteristics is limited. In a ten-year period from 1986 to 1995 at Tri-Service General Hospital, there were 70 small intestine malignancies, accounting for 1.7% of gastrointestinal tract cancers during that time. Among the 70 patients, 25 had adenocarcinomas. Adenocarcinoma was the most common malignancy (35.7%) of the small intestine. The average age at presentation was 62.5 years (range from 49 to 90). There were 16 males and 9 females with a male/female ratio of 1.8:1. The tumors were located more frequently in the duodenum (76%) than in the jejunum (16%) and ileum (8%). The most common presenting symptoms were pain (68%), bowel obstruction (56%), bleeding (44%) and weight loss (36%). Ten cases (40%) had anemia and positive occult blood was found in 14 cases (56%). Sixteen out of 23 cases (70%) who had tumors located at the jejunum and above had undergone endoscopic biopsy all had a preoperative tissue diagnosis. Both abdominal CT scanning and small bowel barium study were highly sensitive in detecting these advanced tumors with a positive rate of 91% and 87%, respectively. Seventeen patients (68%) underwent surgery and only 9 of them had their tumor resected. The surgical mortality was 5.8%. Regional lymph node invasion and distant metastasis were found in 13 (52%) and 10 (40%) patients, respectively, at the time of diagnosis and these may be responsible for the dismal survival rates with 56.5%, 38.1% and 23.5% for 6-month, 1-year and 3-year, respectively. Awareness of the constellation of symptoms may lead to earlier diagnosis and a better outcome for these relatively rare tumors. |
