中文摘要 | 單側睪丸扭轉後之病患,日後可能造成不孕症。此乃因為對側睪丸精子生成功能受到自體精蟲抗體之傷害而引起。此研究擬以睪丸抽取液之去氧核糖核酸細胞圖形(DNA Histogram)來評估當單側睪丸完全缺血時,兩側睪丸精子生成功能之變化。五十隻成熟大白鼠共分成五組;其中一組為對照組,只施以模擬手術(Sham Operation);另外四組大白鼠之睪丸缺血時間分別為三十分鐘,一小時,二小時及四小時不等。其睪丸抽取液經細胞流動圖形儀分析後發現,單套比例(Haploid Ratio)在對照組並無任何變化。但在缺血三十分鐘之大白鼠中其單套細胞在缺血後一小時及一週皆見顯著減少。前者吾人以為可能因缺血引起,後者則可能由免疫反應造成。同側睪丸之單套細胞在缺血一小時以上即可見明顯且持續減少。至於對側睪丸之影響則在缺血二小時後發現,當缺血超過四小時以上則對側睪丸精子生成功能可見持續之誠低。 由以上結果,吾人認為,睪丸抽取液之去氧核糖核酸細胞圖形為追蹤睪丸精子生成功能之一可行檢查。大白鼠睪丸之黃金缺血時間(Golden period)為一小時。若睪丸缺血超過兩小時,則吾人建議此已缺血之睪丸應除去,以免造成對側正常畢丸之傷害。 |
英文摘要 | It has been documented that the patients with testicular torsion would diminish the fertility. The contralateral testis might be damanaged by immunity mediated process. This study wanted to elucidate the DNA histogram change of bilateral testes when a unilateral testis was deprived of blood supply completely. Fifty mature male Sprague Dawley rats were divided into 5 groups. The duration of testicular ischemia were 30 minutes, 1 hour, 2 hours, 4 hours in four groups respectively. Another 10 rats were treated by sham operation. The testis aspirate was analyzed by flow cytometry for their spermatogenesis function. There was no change of haploid ratio in the sham operation group. In the group of testis ischemic for 30 minutes, there were the early decrement of haploid ratio. This is possibly caused by ischemic effect, and the late decrement might be induced by the immunity mediated damage which occurred between 1 day and 1 week. The ipsilateral significant decrement of haploid ratio was found in the testis ischemic for more than 1 hour. The contralateral change occurred in the testis ischemic for more than 2 hours, but the persistent change was noted in the 4 hours group only. In conclusion, the golden period of testicular ischemia in rats is 1 hour. A serial monitor of testis aspiration by DNA Histogram analysis is suggested to decide which testis can be preserved. Those testes being ischemic longer than 2 hours might be removed for fear of inducing damage over the contralateral innocent testis. |
