中文摘要 | 東部某精神專科教學醫院於98-100年度的藥物不良反應通報量平均為38.1件/年,通報件數不盡理想,因此設計本專案,運用跨領域團隊合作方式,結合醫師、藥師、護理師、醫檢師、資訊室等人員,提供各專業領域之建議,共同集思廣義提升院內藥物不良反應通報成效。改善方案為利用要因分析找出通報率不高的原因,並擬定對策:一、進行診間衛教活動、海報宣導、藥袋便利貼、診間門口標語提示,提醒病人若服藥後不適,可主動告知醫護人員,增加病人認知。二、於全院及各部門會議中宣導通報流程與重要性;各護理單位推派專責通報人員、並培育種子教師,以利資訊之傳遞及業務交流。三、定期統計分析院內藥物不良反應通報現況並全院性公告。四、簡化醫令系統通報流程介面,建立與通報者的雙向回饋系統。實施品管圈活動後,101年6-12月的通報案件為113件 (平均:16.1件/月),與100年度同期比較,進步率達151.4 %,本專案活動成效除了件數提升外,也增加醫護人員對藥物不良反應通報的認知與重要性,對此藥事服務產生共鳴。 |
英文摘要 | From 2009 to 2011, a psychiatric teaching hospital in Eastern Taiwan received an average of 38.1 reports per year on adverse drug reactions. To improve on this less than ideal report rate, this study brought together physicians, pharmacists, nurses, medical technicians and IT staff to provide recommendations from their respective fields of expertise and collectively brainstorm how the effectiveness of reporting on adverse drug reactions could be improved. Factor analysis was used to identify the causes of the low report rate, after which countermeasures were formulated. These are listed as follows: 1. Health education activities, poster campaigns, stickers on medicine bags, and slogans are to be used to remind patients to contact medical staff if they experience any discomfort after taking medication. 2. Reporting procedures and the importance of reporting should be promoted in hospital-wide and departmental meetings. It is necessary to designate a dedicated facilitator for reporting in each nursing unit. Local volunteers should be trained to assist in the smooth transmission of information and administrative processes. 3. Regular statistical analysis of report rates should be conducted and published in hospital-wide reports. 4. The reporting interface must be simplified as well as modified to include a two-way feedback system between medical staff and individuals who report adverse drug reactions. After the implementation of these quality control measures, 113 reports of adverse drug reactions were received during June – December 2012 (an average of 16.1 reports per month), an improvement of 151.41% over the same period in 2011. Apart from improving report rates, this project also created greater awareness among medical staff of the importance of reporting adverse drug reactions. |
