中文摘要 | 有效的免疫調節功能和抑制病原體菌叢的增殖,使得益生菌成為未來看好的治療方法之一。然而,臨床試驗的結果卻無法在有無使用益生菌的組別觀察到顯著差異。此一現象可由兩方面解讀:一、致病機轉不只一種,益生菌無法藉由影響其中一個機轉,而達到緩解疾病之目的。二、益生菌的種類和使用劑量在不同研究中差異極大,亦會影響實驗結果的判讀。根據目前的研究成果,益生菌的使用的確無法成為主流療法,但可提供臨床照護者多一類藥物選擇。對於較為棘手或病情較為複雜的病人,或許可以考慮給予適量益生菌,有助於改善疾病狀態。 |
英文摘要 | Probiotics, defined as live microorganisms administered in adequate amounts confer a health benefit on the host by FAO/WHO, are promising agents on both gastroenteral (GI) and respiratory diseases. They can effectively turn the originated and comprimised intestinal microbs into normal and healthy ones. Intact microbiota can further strengthen the function of mucosal barrier, manifesting as intensifying the immune response to xenobiotics or eliminating the toxic pathogen. Its benefits come from effectively altering the mucosal immunity and preventing pathogen from colonizing. Many diseases are related to devastating immune function, including necrotizing enterocolitis (NEC), H. pylori infection, and pneumonia. Theoretically, we can expect the recovery or delayed progression on diseases after administration of probiotics. But, in fact, according to the end points of clinical trials, it is hardly delineated that the probiotics are significantly beneficial on diseases. Complicated pathological mechanisms of various diseases and imperfect study design are suspected the possible causes. In conclusion, the roles of probiotics remain as adjunctive therapy on therapeutic consideration. It is necessary to conduct further studies and well-designed trials to support rational and regular use of different probiotics. In particular, based on the results available so far, pharmacists play an important role on providing correct concepts and intact information to the general population. |
