中文摘要 | 超音波對乳房病變的診斷和乳房攝影一樣重要。很多醫學影像及乳房學專家多年來希望能應用自動化或半自動化的方式來檢查乳房以減少人為的分歧,並希望自動化乳房超音波系統可以和乳房攝影ー樣用來作為篩檢婦女乳房的工具。經過數年努力後,影像學專家發現舊式自動化掃描儀所呈示出来的影像品質不如即時性超音波掃描儀。不過,現今自動化掃描儀所具有的寬頻探頭技術,已能達到足夠的解像力與穿透力而使影像品質大幅改善。加上電腦科技進歩,高速度化的電腦,全體積乳房超音波資料已可迅速獲得,使自動化乳房檢查成為可能。目前乳房體積超音波(volumertic ultrasound)已被視為一種乳房腫瘤診斷的輔助工具。目前的設計以平躺式檢查比直立式檢查較易成功,且已逐漸被接納。這種設計目前習慣被稱做自動化乳房超音波系統(Automated Breast Ultrasound System,ABUS ) (U-System, San Jose, CA)。主要包含一個掃描機座(scan station)及影橡機座(view station)。前者作為自動化掃描乳房之用,後者為影像蒐集、處理、及分析體積資料系統。乳房檢查專家可以分析所有蒐集的資料進行各種不同切面覆閲。最近的研究中比較ABUS及手握式超音波對於乳房病灶的偵測率,對於囊腫性病灶偵測,ABUS可偵測到更多小病灶;惡性腫瘤及纖維腺瘤偵測率可達100%。ABUS提供較好的乳房解剖構造,乳房病灶定位和記錄方式,具很好的可重覆性。根據現今的技術,全自動化系統可以提供更好固質性病灶和癌症的偵測率。ABUS操作簡單,訓練時間短,此一操作技術特別適合技術員完成整個檢查流程。ABUS或許可以做為乳房攝影檢查的轉助儀器。以現今的技術,ABUS 可以作為一個輔助性篩檢工具,協助篩檢婦女緻密性乳房中無法觸摸到的腫瘤。 |
英文摘要 | Ultrasonography (US) has been utilized as an important imaging modality in the diagnosis of breast diseases. Automated full-field breast US ( AFBUS ) scanners were originally designed to effectively examine the breast in its entirety, and had been proposed to be used for screening breast cancer by some sonologists and breast imaging experts. However, the early efforts were not successful because of the relatively poor resolution. Currently, automated scanners equipped with high frequency broadband transducers are significantly compared with hand-held instruments. With the rapid development of computer technologies, volumetric ultrasound can be now applied in the breasts. Volumetric ultrasound scanning of the breast has been proposed as a complimentary modality for breast cancer screening. The new supine-type compression ABUS scanning apparatus ( U-system, San Jose, CA) comprises a rigid and substantially stationary frame and a compressive membrane ( a polyester film sheet) . By manually pushing the frame to contact firmly to the relatively soft breast, the transducer can optimally scan the breast if sufficient amount of gel is evenly applied on the breast surface, leaving minimal or no contact artifacts. Interpretation of data obtained from the volumetric breast ultrasonographic imaging should include reviewing the SomoV and Standard Vu. With current volumetric technologies, the ABUS system can provide high detection rate of infiltrative ductal cancers and fibroadenomas ( 100%) based on our recent experience. The current ABUS scanners are easy to use and needs only a short period of training, and the examination technique is especially suitable for technologists or sonographers to perform the whole scanning procedure. ABUS may be used as an adjunct to MG or a baseline US examination of the breast and can play an important role in screening nonpalpable tumors in women with dense breasts. |
