中文摘要 | 臨床上老年人的多重用藥並不少見。87歲的張先生由於服用的藥物種類太多而至家醫科門診求診。其口服藥分別來自 6個專科處方,單日品項高達21種口服藥,總數目達43.5顆及6種外用藥。 由於老人的藥物吸收、分佈、代謝及排泄與年輕人不同,令其較易發生藥物不良反應;其中多重用藥是重要風險因子之一。臨床上Beers準則為最常用的老人用藥指引。本案例之用藥品項並未重覆,也未嚴重違反Beers準則,但如此多種藥物在老年人體內所可能產生的藥物交互作用,仍不可輕忽。同時,老年人用藥需要有「醫療守門員」為其把關,值得老年專科醫師省思。 |
英文摘要 | Polypharmacy is especially common in the elderly. Take for example Mr. Chang, an 87-year-old man, who presented in the outpatient clinic of the Department of Family Medicine with the chief complaint of too many medications. His oral medications were prescribed from 6 specialties, with a total of 21 different oral medicines comprising of 43.5 oral pills to be taken daily, in addition to topical medications. Since the absorption, distribution, metabolism and excretion of medications differ in the elderly compared to younger people, they are especially more prone to adverse drug reactions (ADR). Polypharmacy is an important risk factor to ADR. Although the medications used in this case neither have any repetition nor violated Beers criteria, the most widely used prescription guidelines for the elderly, the large number of potential drug-drug interactions still warrant concern. Meanwhile, Gerontology specialists or geriatricians should play the roles of gatekeepers for medication use in the elderly as well as lifestyle modification. |
