中文摘要 | 病犬為一年約6月齡,體重8公斤未絕育雄性混種犬,飼主認養該犬後發現其運動不耐,於某獸醫教學醫院診斷為肺動脈狹窄並開立口服藥物atenolol,為進一步積極治療,遂帶至本校獸醫教學醫院就診,理學檢查於右側胸壁可聽見第三級(Ⅲ/Ⅳ)心雜音,放射線學及心電圖檢查顯示右心室擴大,心臟超音波學檢查可見肺動脈狹窄,右心室與肺動脈壓力差為106mmHg,初步診斷為犬之先天性瓣膜性肺動脈狹窄。於兩週後進行心血管照影檢查及氣球擴張術,術後聽診無心雜音,右心室與肺動脈壓力差降為368mmHg。術後第46天回院複診,聽診發現病犬有第二級(Ⅱ/Ⅳ)心雜音,為病犬進行心臟超音波學檢查發現肺動脈再狹窄,右心室與肺動脈幹壓力差為924mmHg,暫以內科治療,同時囑咐飼主替病犬限制運動。最終診斷為犬之先天性瓣膜性肺動脈狹窄。 |
英文摘要 | A six-month-old, non-neutered male, mixed-breed dog that weighed 8 kg was referred for evaluation of exercise intolerance. The dog was diagnosed with pulmonary stenosis at one of the veterinary teaching hospital in Taiwan and was treated with atenolol. The dog was presented to the National Chung Hsing University Veterinary Teaching Hospital 2 month later for further treatment. Upon physical examination, a grade Ⅱ/Ⅵ murmur on the right side of thorax was detected. Radio-graphs and eletrocardiographic evaluations showed enlargement of the right ventricle. Echocardiographic examinations revealed a pressure gradient of 106 mmHg between the right ventricle and the pulmonary artery. A tentative diagnosis of congenital valvular pulmonic stenosis was given to the patient. The dog underwent angiocardiography and balloon valvuloplasty two weeks later. After the surgery, heart murmur was no longer detected and the dog showed a decrease of the pressure gradient to 36.8 mmHg. However, 46 days after the intervention, grade Ⅱ/Ⅵ heart murmurs were detected and echocardiography had revealed an increase of the pressure gradient to 92.4 mmHg. A definitive diagnosis of congenital valvular pulmonic stenosis in a dog was made. The dog is currently under therapy and exercise is restricted. The case is still under observation. |
