中文摘要 | 使用電腦斷層攝影來評估心血管疾病是現今醫學中最重要的非侵入性方式。近年來,新一代能譜電腦斷層掃描儀在偵檢器材質及影像處理技術上有很大的改良,但我們仍對於該掃描儀在心臟冠狀血管電腦斷層攝影上的輻射劑量存有疑慮。過去文獻曾提及在傳統多排電腦斷層掃描儀的檢查結果中,BMI之高低與性別的差異在劑量上會有顯著的差異,而不同年齡層的差異則沒有此現象。為了評估此兩種掃描儀在心臟冠狀血管電腦斷層攝影上的劑量差異,我們採用三個身體素質因子(BMI、年齡、性別)分別探討各因子在不同掃描儀上之劑量差異。結果顯示,與傳統多排電腦斷層掃描儀相比,新一代能譜電腦斷層掃描儀在BMI大於24kg/m^2的受檢者群組中較有明顯的劑量降低,其大幅降低41%的輻射劑量;在不同年齡與性別的類別群組中,新一代能譜電腦斷層掃描儀的劑量皆有較低之劑量結果。並利用四因子變異數分析得知,該檢查的劑量會受到掃描儀、BMI、及掃描儀與BMI的交互作用而有差異,但不會受到年齡、性別、掃描儀與年齡的交互作用、及掃描儀與性別的交互作用而影響。 |
英文摘要 | Cardiac coronary computed tomography (CT) scan is an important and non-invasive method to assess cardiovascular diseases. Recently, the novel spectrum CT scanner have replaced the conventional multi-slice CT scanner, because the novel spectrum CT scanner could provide lower radiation dose and better image resolution. A previous study demonstrated that, using the conventional multi-slice CT scanner, the different BMI and different gender significantly affect the cardiac coronary CT dose; however, the different age also could change the dose, which didn't reach statistical significances. The aims of this study are to estimate the influences of BMI, age, and gender on radiation dose in these two scanners. Our results showed that, compared with the conventional multi-slice CT scanner, the novel spectrum CT scanner in a group with BMI greater than 24 kg/m^2 had significantly lower dose (i.e., in 41% dose reduction). The similar results also found in different age and gender groups. Additionally, the four-way analysis of variance (ANOVA) was used to estimate the main and interaction influences on scanner, BMI, age, and gender. The results showed that the different scanner and the interaction between scanner and BMI could significantly influence cardiac coronary CT dose; whereas the other main and interaction didn't affect the dose. |
