中文摘要 | 背景:核醫鎝-99m標幟紅血球靜脈攝影((上標 99m)Tc-RBC venography)在攝影小腿靜脈時,一般建議病人以俯臥姿造影,使血管與偵檢器的距離縮短,但對於無法翻身俯臥的病人,只能採用仰臥姿,並將偵檢頭朝上由床下往上攝影。因病人採用仰臥姿勢時,小腿放在床上會擠壓小腿肌肉。本研究評估小腿在有無壓迫姿勢下,對核醫鎝-99m標幟紅血球靜脈攝影之影響。方法:本研究收集20位疑似深部靜脈栓塞之病人各進行二組姿勢的造影,第一組爲俯臥姿勢,第二組爲小腿貼放在床上之壓迫姿勢。二組影像交由核醫科專科醫師判讀,將兩腿之膕靜脈、腓靜脈及後經靜脈依其放射性分佈分爲三級:正常、輕微減少、中等減少,再將資料作統計分析比較。結果:在腓靜脈部分,俯臥姿勢出現中等減少有7%,而壓迫姿勢會因貼放在床上擠壓肌肉使腿部形狀變寬導致血管出現中等減少較多有27%。在後經靜脈部分,俯臥姿勢出現中等減少有12%,仰臥姿勢出現中等減少較多有40%。小腿肚的壓迫確實會造成核醫鎝-99m標幟紅血球靜脈攝影判讀小腿靜脈時,僞陽性明顯增加。結論:進行核醫鎝-99m標幟紅血球小腿靜脈攝影,最好避免採用仰臥姿勢,減少小腿肚的壓迫,以得到正確的深部靜脈栓塞(DVT)診斷。 |
英文摘要 | Background: 99mTc-RBC venography is usually performed in prone posture in order to decrease the distance between the collimator and calves vessels. Supine posture is used with the scanner placed under the examination table when patients are not able to conduct prone position. However, compression of calf vessels is inevitable and may weaken the diagnostic accuracy when supine posture is performed. The aim of this study was to evaluate the influence of difference imaging postures on 99mTc-RBC venography of lower limbs. Methods: 99mTc-RBC venography was carried out in 20 patients in whom deep vein thrombosis was suspected. All patients were analyzed negative for deep vein thrombosis over calves. The two postures (prone, supine) were compared on the basis of individual vessel segments over calves. Results: There is no influence over the popliteal veins among different postures. In peroneal vein segment, moderate decrease in 7% and 27% of patients when prone and supine postures, respectively. In posterior tibial vein segment, moderate decrease in 12% and 40% of patients when prone and supine postures, respectively. Conclusion: Prone posture is the best method to detect vessels of calves in 99mTc-RBC venography. |
