中文摘要 | 背景:腋動脈斷裂為罕見的肩關節脫臼之嚴重併發症,於整型外科的文獻中,目前尚未有相關的病例報告。肩關節脫臼若併發腋動脈斷裂,會產生類似臂神經叢損傷的症狀,進而延遲發現的時機。若無及時診斷出腋動脈斷裂,則可能因缺血造成嚴重的功能損失,甚至截肢。目的及目標:案例報告一位74歲男性肩關節脫臼後併發腋動脈斷裂的臨床表現、影像學檢查、手術探查治療以及預後情形。材料與方法:一位74歲男性,由高處墜落時造成左肩關節脫臼,經徒手復位後出現左上肢腫脹、發紫、麻痺等症狀,血液循環不佳,經血管攝影及手術探查發現腋動脈於第三部分完全斷裂。結果:經手術修補後左上肢循環恢復,感覺及運動功能亦於追蹤後逐漸復原。結論:腋動脈斷裂或損傷在肩關節脫臼中是少見的相關併發症,迅速及時的診斷及手術探查才能達到最好的預後。 |
英文摘要 | Background:Axillary artery disruption as a complication of shoulder dislocation is a rare event and has never been reported in the plastic surgery literature. Symptoms may mimic those of a brachial plexus injury with sensory or functional deficits. When brachial plexus injuries are suspected, the plastic surgeon is often consulted to perform surgical exploration. When diagnosis of axillary artery disruption is delayed or missed, consequences may be detrimental resulting in permanent disabilities or limb loss.Aim and objectives:We presented one patient had acute shoulder dislocation, which complicated with axillary artery disruption. The aim of this report is to display the clinical course, image study, surgical management and prognosis.Materials and methods:One 74-year-old man suffered from left shoulder closed dislocation because of falling down. After closed reduction, left upper limb swelling, ecchymosis, numbness and weakness gradually developed. Computed tomography (CT) angiography showed complete disruption of axillary artery and emergent surgical exploration and repair were performed.Results:Post-operative left upper limb circulation was recovered well. The sensory and motor function also returned to adequate level.Conclusions:Given the high risk for patient morbidities, it is important that appropriate diagnosis and intervention be carried out in a timely manner. Therefore, it is necessary that plastic surgeons are made aware of this grave complication in the setting of shoulder dislocation and possible brachial plexus injuries. |
