中文摘要 | 植物人狀態(vegetative state)乃因大腦兩半球極度嚴重的損害或功能障礙所造成的自覺的思維活動能力的喪失,但間腦與腦幹的功能有充裕的保留,因此一些自主神經反射與運動反射都有保存,並可有正常的睡眠-覺醒週期。本例為頭部外傷導致廣泛性軸突損傷及腦幹梗塞,致功能喪失。頭部外傷所致植物人狀態復健之黃金時期為受傷復三個月內,這段期間大部分在住院。經由西醫會診,對頭部外傷所致植物人狀態住院患者進行中西醫結合治療,中醫部分以針灸治療為主,西醫部分依常規治療,本例的昏迷指數及巴氏量表由會診前的9分及10分,進步到治療復的15分及100分,療效顯著。若能建立中西醫結合治療的臨床路徑,更能掌握治療的黃金時期,進而提昇療效並縮短療程,使醫療成本降低。 |
英文摘要 | A vegetative state is prolonged unresponsiveness in a state of wakefulness without awareness. It is due to overwhelming dysfunction of the cerebral hemispheres, with sufficient sparing of the diencephalon and brain stem to preserve autonomic and motor reflexes and sleep-wake cycles. This case was caused by diffuse axonal injury and brain stem infarction following her head injury. The golden period for recovery from vegetative state due to head injury is 3 months after the episode which is during admission among most cases. We treat the patients in vegetative state due to head injury with the combination of Western and traditional Chinese medicine (WM & TCM) via the consultation from western medical department. The patients treated in TCM are major with acupuncture and in WM are routine. The therapeutic effect is excellent in this case; her GCS/Barthel's index was getting from 9/10 to 15/100, before and after treatment. We can promote the therapeutic effect, shorten the course of treatment, and reduce the medical cost if the clinical pathway of therapy of combining WM & TCM is set up. |
