中文摘要 | 近視性黃斑部病變可以造成失明是高度近視最重要的併發症。近視度數 和眼軸長是影響黃斑部病變嚴重性最主要的兩項因素.另外,由黃斑部的組織病 理學可看出脤絡膜血管的阻塞,而年齡因素被認為是造成小血管阻塞的主因。所 以,我們收集台大高度近視門診癌患811名(計1601眼)來評估屈光度、眼軸長和年 齡與黃斑部病變之關係。結果顯示黃斑部病變之進行由T�窗覾�砥AT�砥羇�湄 �窗羇�祟M屈光度之增加及眼軸延長有關。近視屈光度超過-15.00及眼軸長超過 29.0mm是產生初期黃斑鄧萎縮(D1)之危險因子。相對地,由P�窗蠪�砥AP�砥蠬A 之進行和屈光度之增加及眼軸之延長無關。光是年齡老化因素即可造成黃斑部病 變之進行,所以脤絡膜之缺血可能是造成黃斑部病變的主要機轉。(慈濟醫學 1995;7:29-37) |
英文摘要 | Myopic maculopathy is one of the major complications in high myopia which can cause blindness. Refractive error and axial length are two main factors that affect the severity of maculopathy. Besides, the histopathogenesis of macular area showed obliteration of choriocapillary and aging factor was suggested to involve inthose small vessel obstruction. Therefore, we collected and classified the 811 patients (1601 eyes) from highmyopic clinic of National Taiwan University Hospital to evaluate the relation between refractive error, axiallength and aging factor with the severity of maculopathy. Results showed that the severity of maculopathy fromT�� to T��, T�� to D��, D�� to D�� were correlated with increasing of refractive error, axial elongation and aging.Refractive error over -1 5.0D and axial length longer than 29.0 mm are two risk factors for the early myopicmaculopathy (D ). In contrast, the severity of maculopathy from P�� to P��, or P�� to MA were not correlated withthe increasing of refractive errors or axial elongation. However, only aging factor (older than 40 y/o), neitherincreasing of refractive error nor axial elongation, would cause the progression of maculopathy. Thus, agingcause the ischemia of choroid may be a major mechanism in myopic maculopathy. (Tzu Chi Med J 1995; 7:29-37) |
