中文摘要 | 背景:在實驗性的局部或全心心肌缺血後的早期再灌流損害中,以人參作為拮抗劑的研究未見報告。 材料和方法:20隻實驗犬隨機分為兩組,局部和全心缺血再灌流損害模型(1組和2組),並進一步分為四個組;兩個對照組(1A組和2A組),兩個實驗組(1B組和2B組),每個組均有實驗犬5隻。1組結紮左冠狀動脈前降支45分鐘;2組接受常溫體外循環阻斷上行主動脈45分鐘。以轉子泵將10ml/kg復方人參注射液輸入上行主動脈內,持續1小時。採取缺血區域的左室前壁組織,分別進行蘇木素-伊紅染色、糖元染色和琥珀酸去氫(SDH)染色。 結果:蘇木素-伊紅染色:在第1組,對照組的缺血期和再灌流期,心肌結構分別呈輕和中度的變性;實驗組的兩期內心肌結構均基本正常。在第2組,對照組缺血心肌仍呈良好結構,而再灌流的心肌則呈輕度變性。實驗組,缺血及再灌流心肌細胞均呈現基本正常的結構。糖元染色:在第1組,對照組兩期內糖毛染色為弱陽性、而實驗組的兩期內則呈中等度的陽性。在第2組,對照組和實驗組分別為陽性和陰性的染色結果。組內兩期的比較有顯著性的差異。SDH染色:在第1組,對照組的SDH染色為陰性;實驗組的心肌SDH呈現藍染顆粒,再灌流期較缺血期更強。在第2組,兩期的SDH染色均呈強陽性,1A和1B組的兩期間各自的比較有顯著的差異。多因素回歸分析發現再灌流期左心室的功能顯著地受到負相關因素心肌水腫的厲害程度、心肌糖元含量和正相關因素心肌細胞內SDH活性的影響。 結論:心肌損害在局部缺血再灌流模型較全心心肌缺血再灌流模型厲害;在再灌流期較缺血期厲害。復方人參注射液對心肌缺血再灌流損害具存保護作用,其可能的機轉是保存心肌細胞內糖元的含量和SDH活性。 |
英文摘要 | Background: Assessment of Radix Ginseng myocardial cytochemistry against reperfusion injury of early stages after either experimental regional or global ischemia has not previously been studied. Materials and Methods: Twenty mongrel dogs were divided randomly into two groups: the regional and the global ischemia models were Groups 1 and 2, then further divided into four subgroups; there were two control (Subgroups lA and 2A), and two experimental (Subgroups lB and 2B) with five animals in each. Coronary ligation was used in the regiona Land normothermic cardiopulmonary bypass in the global, models, respectively. Injectio Radix Ginseng Compositae was given continuously by a roller pump for one hour. Myocardial biopsies were taken from the anterior wall of the left ventricle. Hematoxylin and eosin staining and cytochemistry of glycogen and succinate dehydrogenase (SDH) were assessed. Results: Heinatoxylin and eosin staining. In Group 1, the myocardial structure showed slight and moderate degeneration in the ischemic and reperfusion periods of the control, respectively, and was nearly normal in both periods of the experimental subgroups. In Group 2, the ischemic myocardium still showed good structure, and the reperfused revealed slight degeneration in the control; the ischemic and reperfused myocardial cells were normal in the experimental subgroup. Glycogen staining. In Group 1, glycogen was stained weak positive in the control, but moderate positive in the experimental subgroup in either the ischemic or reperfusion period. In Group 2, it was stained positive in the control, but negative in the experimental, subgroup. Significant differences between periods were found. SDH staining. In Group 1, SDH was stained negative in the myocardium of the control subgroup. And the myocardium was stained blue. stronger in the reperfusion than in the ischemic period of the experimental subgroup. In Group 2, SDH staining of the two periods showed strong positive. Significant differences were found in Subgroup 1B versus 1A in both periods. Conclusions: Myocardial injury showed more severe in the regional than in the global model, and in the reperfusion than in the ischemic periods. Injectio Radix Ginseng Compositae seems to have a preservation effect on both ischemia and reperfusion injuries through a probable mechanism of preserving myocardial glycogen content as well as SDH activity. |
