| 中文摘要 | 頸椎損傷個案困生理上的功能癱瘓,導致心理壓力而產生自我照顧能力缺失、皮膚完整性受損、言辭溝通障礙、呼吱道清除功能失效、潛在危險性廢用症候群及無力感都是首要且必須解決的問題。在協助個案接受壓力引起的無力感及無望行為以及社會功能的退化等,這些都是會引起照顧者的壓力,亦會造成社會上的成本負擔,故如何協助頸椎損傷個案能接受疾病發生的事實,進而配合做復健治療,是護理人員及醫療團隊的一大挑戰。本報告是探討一位因車禍導致第四、五頸椎損傷個棄之護理經驗。護理期間自92年1月30至92年2月28日,筆者經由觀察、身體評估、病歷查閱及與家屬、醫療團隊討論、會談方式收集資料,並利用羅氏適應模式進行評估,發現個案主要健康問題有:呼吸道清除功能失效、潛在危險性肺吸入、便秘、完全性尿失禁、潛在危險性感染及潛在危險性廢用症候群,如何建立個案信心進而提昇生活品質,使其達到最佳之健康狀態是護理期間迫切需要的,此護理經驗顯示對個案進行各方面的詳細評估及醫療團隊的配合,皆是使頸椎損傷個案能接受其疾病過程的重要因素。 |
| 英文摘要 | That helplessness, hopelessness, and social function deterioration induced by the physical paralysis and mental stress of a patient with cervical spinal injury would not only result in difficulties for the caregiver, but also increase socioeconomic burden. As a result, helping the patient to accept the occurrence of this injury and further cooperate with the rehabilitation therapy is a challenge for nurses and the medical team. This report is to share the nursing experiences in caring for a patient with 4th and 5th cervical spinal injury that resulted from a traffic accident. The duration of nursing care was from 30th January 2003 to 28th February 2003. The authors gathered information through observations, physical assessments, medical chart reviews, interviews with family members, and discussions with the medical team. The authors also applied Roy's Adaptation Model for evaluation of the patient and found several critical physical problems of this patient, including failure in respiratory tract clearance function, risk of aspiration pneumonia, constipation, urine incontinence, risk of infection, risk of wasting syndrome, deficiency in personal care, deficiency in skin integrity, helplessness, and verbal communication disorders. The authors then found that failure in respiratory tract clearance function, risk of wasting syndrome, and helplessness were the problems that required primary and utmost solutions. Assistance to help the patient accept the current condition, rebuild faith, and further improve living qualities was crucial and urgently needed during the nursing period, so that the patient could achieve the healthiest physical status. These nursing experiences showed that careful evaluation on patient's needs in every aspect as well as the cooperation of the entire medical team were both important factors to help the patient with cervical spinal injury accept his/her current ailment. |
