英文摘要 | Purpose. To evaluate corneal endothelial reserve in senile cataract patients previously treated with a glaucoma filtering operation. Methods. During a three-month study period, a total of 36 senile cataract patients (58 eyes) were enrolled for assessment of the corneal endothelial morphology. Of these, 26 eyes of 20 patients had undergone a filtering operation elsewhere more than four years previously (9.2 ± 6.0 years, mean ± standard deviation) for angle-closure glaucoma. The remaining 32 eyes of 16 patients with primary angle-closure glaucoma had never been operated and thus served as the controls. Specular microscopy was performed on all patients to evaluate the endothelial cell density (ECD), mean cell area (MCA), coefficient of variation (CV) of cell area and percentage of hexagonal cells. Results. Patients previously treated with a filtering operation had a significantly lower ECD; 2173 ± 731 cells/mm �� vs 2821 ± 300 cells/mm �� ,higher MCA; 527 ± 216 μ m �� vs 358 ± 40 μ m ��, and higher CV of cell area, 0.323 ± 0.065 vs 0.285 ± 0.063 (p<0.05). Percentage of hexagonal cells was lower; 59.9 ± 8.4% vs 62.8 ± 6.5%, although it was not statistically significant. Conclusions. Angle-closure glaucoma patients previously treated with a filtering operation have a relatively compromised corneal endothelium characterized by polymegathism. Specular microscopic examination should be performed routinely in these high-risk patients prior to a cataract operation. |
