中文摘要 | 近代社會快速的變化,造成社會大眾對醫界有不同的期求與不斷衝突的發生,這也顯示出以往的醫學教育有某些方面的缺失或不足,這些現象已促使醫界有識之士們的自省,進而從世界各地與臺灣皆積極進行著醫學教育改革。但是,另外一個問題是,教育改革真的教出更多的好醫生嗎?醫生們是否比較能以病人為中心、關心病人,現在則還看不出來;但醫病關係及在醫德方面的提升,要透過小組教學來達到成效,院讓多數教師們感到難以使上力,而這也是臨床醫師們現在服務面臨最大的難題。本文從臨床的經驗,從一些常得面臨的情境,探討醫療倫理原則與面臨的問題;如臨床試驗與病患自主權(Autonomy),知情同意(Informed Consent)、誠信原則;醫主原則代行病患自主權與不良的溝通;行善與不傷害原則(Beneficence and non-maleficence)與醫療友誼的誼立(Medical friendship);公平原則與醫師的退縮。理想與實際的差異,借這些情境衝擊來思索、認知醫療教育與倫理和現行醫療環境的可能衝突,以提供讓大家可進一步深思,期望以後醫學教育與醫療倫理的探討與進展。 |
英文摘要 | In this fast track society, people have developed different expectation towards the medical industry. The presence of more and more confronations between health caregivers and patients suggested the presence of defaults and deficiency in our past medical educational program. This urged leaders of medical education to put effects in reforming the educational program. Since then, new questions are raised on how good these reforms are in producing better doctors. Presently it is difficult to judge whether doctors educated from the new program are more patient centered and patient caring. However as lessons for doctor-patient relationship and medical ethics are given through didactic group learning, most tutors have found difficulties in passing the true message to the students. The following review, based on common clinical experiences is focus on the principles of medical ethics and their related problems. Issues on clinical experiment, autonomy, informed consent, beneficence, non-maleficence, justice and medical friendship are addressed. We aim to provide readers a better understanding of the conflicts between medical education and our present medical environment and on the differences between idealism and reality of medical education. Our goal is to stimulate readers to consider more on decoding medical ethic into their daily medical practice. |
