英文摘要 | External cephalic version has been advocated for breech presentation at or near term to reduce perinatal mortality and morbidity and the need for cesarean births. With the use of a tocolytic agent and careful patient selection the technique of external cephalic version (ECV) has become much safer with a higher rate of success when applied to patients with breech presentations at a gestational age of 36 weeks or greater. From Nov. 1989 to Nov. 1991, a prospective study of ECV on 49 patients with breech presentations at a GA of 36 weeks or greater was done. Among the 49 patients who agreed to perform ECV two births resulted in spontaneous versions and 47 births resulted from ECVs performed under tocolysis. The overall successful rate was 79% (37/47); the rates for nulliparous and multiparous birth were 71% (17/24) and 87% (20/23), respectively. The cesarean section rate was 11% in the successful group and 90% in the unsuccessful group. There were no serious maternal or fetal complications associated with the attempts at version or delivery. External cephalic version was found to be a safe and effective procedure in our study. |
