| 英文摘要 | The benefit of a near-total laryngectomy is that it preserves more normal tissue for organic function. This voice preservation procedure provides a method by which the entire larynx is resected except for a normal periarytenoid strip and a homolateral recurrent laryngeal nerve, with which a dynamic speaking or tracheo-pharyngeal (T-P) shunt is created. We report the use of rhinomanometry and speech analysis system to evaluate the T-P shunt of a patient who had undergone a near-total laryngectomy. When the airflow measured at a sample pressure point was 75 Pa, the resistance was 3.35 Pa/cc/sec during inspiration, and 5.62 Pa/cc/sec during expiration. With the measuring at point 100 Pa, the resistance was 3.71 Pa/cc/sec during inspiration, and 5.38 Pa/cc/sec during expiration. When the measuring point was up to 150 Pa, the resistance was 4.07 Pa/cc/sec during inspiration, and 4.14 Pa/cc/sec during expiration. Fundamental frequency was 366.45 Hz. Jitter was 3.04%. Shimmer was 9.87%. Harmonic to noise ratio was 16.13 dB. Maximal phonation time was 10.9 sec. |
