中文摘要 | 臨床上,齒頸部窩洞對於臨床醫師或病人一直是很大的困擾;對病人而言,一方 面影響美觀, 二方面容易敏感( Hypersensitive ), 甚至易堆積食物, 引起齲齒( caries )及牙周疾病( periodontal disease )。對於牙醫師而言,齒頸部的窩洞並並不 好處理。以前一直認為,齒頸部的窩洞是由於刷牙或由食物中的酸所造成的;近年來發現, 並非所有齒頸部的窩洞都是由刷牙或酸所造成,應力可能是非齲齒性齒頸部窩洞所致病之主 要因素。因此本研究利用有限元素法針對小臼齒( premolar )的不同咬合點及咬合方向進 行齒頸部應力分析。結果發現齒頸部窩洞的生成應與張應力有密切關係,且可能是因長期動 態負載的疲勞現象所導致,同時從分析所得的最大主應力方向與微裂紋破壞方向相互垂直, 可進一步說明為何窩洞多為楔形之故。研究結果顯示臨床的治療方式除了必須對窩洞填補外 ,更應進一步調整咬合點及咬合方向,才能真正達到治療之目的。 |
英文摘要 | Dental cervical lesion is a common finding in clinical dentine. This lesion not only affects the appearance but also hypersensitivity. Toothbrush abrasion and acid erosion have been considered as the causes for the lesion. However, in recent years, stress-induced cervical lesions are widely discussed. In this study, a two-dimensional finite element model of a maxillary second premolar was developed to investigate the stress in the cervical region. Results of the stress analysis indicated that tensile stress of the fatigue failure might be a major factor for dental cervical lesion. The microfracture direction was perpendicular to the maximum principle stress which enhanced the stressinduced lesion conclusions. From these results, effective clinical treatments of dental cervical lesion should include adjusting the occlusion location and direction other than just filling the area of lesion. |
