中文摘要 | 化學治療是癌症病童目前的希望,可是化學治療的副作用卻深深困擾病童及家長,其中對腸胃道的影響最常見,且與營養攝取息息相關。病童之營養狀態會影響其對治療的耐受力及療程是否能順利進行。症狀量表得分可推估四種程度的營養不良,分別為正常(9-10分)、輕度(7-8分)、中度(4-6分)及重度(3分以下)營養不良。本研究目的為用症狀量表連續評估住院接受化療之癌症病童的營養狀態,以及早確認其營養不良之嚴重性並提供改善方案。研究者每天早上測量病童之體重、並依症狀量表評估藥物之副作用。初步資料分析發現16位病童的住院天數為3-25天(眾數為四天)。由症狀量表總分發現癌症住院病童營養不良狀況隨著住院天數增加而日趨嚴重,住院第三天及第四天病童之營養不良人數達60%以上。其中病童有食慾減低、嘔吐、疲倦及腹瀉等情況發生,且隨著住院日數增加而增加。經由症狀量表之評估,能對癌症病童接受化療的營養狀況更加瞭解,可及早提供營養改善措施,減少營養不良的嚴重程度。 |
英文摘要 | The purpose of this ongoing study is twofold: 1) to early identify the nutritional status of hospitalized children receiving cancer chemotherapy; 2) to develop the appropriate strategies to improve their nutritional conditions. The preliminary results including 16 hospitalized children who received a variety of chemotherapy protocols. Demographic data, body weight, and symptoms related to cancer chemotherapy were assessed within 24 hours after admission and every morning during hospitalization. Most of children hospitalized for four days (ranging 3-26 days). Total symptom scores of each child were identified every day. The strategies of nutritional management were recorded. Data were analyzed by descriptive statistics through SPSS/W9.0. Everyday nutritional status of each child was identified according to the symptoms scoring system (Attard-Montalto et al., 1998). The percentage of children with nutritional status change was from 12.5 to 60% over the first four days. The longer the stay of hospitalization, the more the children got loss of appetite, vomiting, diarrhea, and lethargy. The results could add to our understanding of nutritional status in children receiving cancer chemotherapy and early intervention to prevent malnutrition. |
