英文摘要 | Background: Double-orifice mitral valve is a rare cardiac anomaly, usually discovered incidentally during surgery or autopsy. The aim of this study was to delineate the echocardiographic features of this anomaly before direct anatomic observation. Materials and methods: Between January 1993 and December 1997, seven patients (four male and three female), ranging in age from two months to 59 years, were identified as having this anomaly by two-dimensional color Doppler echocardiography. the configuration of the dual mitral orifices were recognized and analyzed. Results: Three morphologic variants were observed: a complete bridge type, where a bridge of fibrous tissue divided the mitral valve into two orifices of equal or unequal parts at the annular level; an incomplete bridge type, where the division was at the leaflet level; and a hole type with the second opening seen only at the mid-leaflet level. Mitral regurgitation occurred in all patients, complicated by significant mitral stenosis in one. Associated intracrdiac malformations included prolapsing mitral valve, ostium primum and secundum defects, and subaortic ventricular septal defect. Conclusion: the echocardiograpic features of double-orifice mitral valve appear to be unique and allow early detection by ultrasonography. The appearance of two discrete orifices at different planes on ultrasound is diagnostic. The parasternal shortaxis views, sweeping from the base toward the apex of the heart, are best for demonstrating any double inlet nature of the mitral valve and for delineating the different types. Visualization of two separate mitral inflows entering the left ventricle in the apical four-chamber view by color flow mapping is an indication to take this anomaly into consideration. |
