英文摘要 | Transient mydriasis and monocular blindness are rare complications during the Wada test. The pathophysiology remained controversial. Data from morbid angiogram and electrophysiologic examinations in such aptinets has been scarce. We present a 28-year-old man who was admitted for temporal lobectomy in such lpatients has been scarce. We present a 28-year-old man who was admitted for temporal lobectomy due to medically intractable seizures. The Wada test was arranged for pre-surgical evaluation. Acute onset of left monocular blindness with pupil dilatation after bolus injection of amybarbital was noted. Morbid angiogram showed normal ophthalmic artery and central retinal artery flow and reduced choroids vascularity. Complete recovery occurred within 1 hour after intra-arterial heparin infusion. Follow-up fundus fluorescein angiography and visual evoked potential were intact. Transient mydriasis and monocular blindness in our patient may have been caused by gas emboli during amybarbital bolus injection. We speculate that reduced choroids vascularity form posterior ciliary artery occlusion could account for the concurrent both optic nerve and ciliary body ischemia. We also suggest that intra-arterial (IA) heparin may be beneficial when such complications occur. |
