英文摘要 | Forty-one patients with isolated brainstem infarct receiving both magnetic resonance imaging (MRI) and MR Angiography (MRA) examinations were analyzed respectively from September 1993 to September 1996 in a period of three years. MRA showed significant vascular lesion such as occlusion and stenosis in vertebrobasilar artery (VBA) in 24 patients: 18 in vertebral artery (VA), 4 in basilar artery (BA), 1 in both VA and BA, and 1 in posterior inferior cerebellar artery (PICA). Large vessel infarcts such as atherosclerosis and embolic origin are the potential pathogenesis in these 24 patients. The other 17 ones could be related to both lipohyalinosis and local arteriosclerotic disease, leading to occlusive disease of penetrating arteries. There is a close correlation beteen stroke in evolution and abnormal vascular lesions demonstrated by MRA, especially in the paramedian pontine terrirory. If MRA reveals unilateral intracranial vertebral artery (ICVA) or BA stenosis in patients with brainstem infarcts, evolving stroke should be cautiously considered. Further data collection is needed for better understanding of the pathogenesis of vertebrobasilar stroke, and providing a more scientific basis for clinicians in therapeutic options. |
